‘Application for a Driver Qualification Card (DQC) for EU/EEA driving licence holders’

This DQC4 form is to be used if you hold an EU/EEA Driving Licence that was not issued in the United Kingdom (UK), and you are applying for a new, renewal or replacement Driver Qualification Card (DQC).

This form must be printed in colour and A4 size.
